2025 Annual Report for the Office of Health Outcomes

Grady’s Office of Health Outcomes focuses on improving access to quality health care, reducing health disparities, and advancing health equity for the communities we serve. Its Annual Report for the Office of Health Outcomes outlines the progress made over the past year.

Key results include:

  • 15% increase in cancer screenings

  • Engagement of 50,000+ individuals across more than 600 community-based events

  • Strong performance across core quality and access measures

  • Expansion of community-based access points

  • Continued growth of our workforce pipeline through TELP and work-based learning

Importantly, this work moves beyond outreach. Grady has strengthened the connection between community engagement and care delivery – ensuring individuals are not only reached but effectively connected to services and follow up care.

The report also outlines key priorities ahead, including strengthening primary care performance, expanding access in high-need areas and scaling workforce development efforts.
