Keeping seniors thriving: Grady's 4Ms of care

September is Healthy Aging Month, a time to focus on how we can all stay healthy as we grow older. At Grady, our Senior Health Care program provides personalized care that keeps seniors active and independent for as long as possible.

Central to Grady’s senior care are the 4Ms of Age-friendly Care, which ensure care is always person-centered, safe and effective with older adults and their families engaged in every decision. This includes:

  • What Matters: Identifies and aligns with what is important to the patient – their goals, preferences, values and priorities outside their disease

  • Medication: Ensures all medications are necessary, safe and do not interfere with what matters, mentation or mobility

  • Mentation: Supports mind and mood by addressing mental health needs

  • Mobility: Promotes safe, active movement to maintain strength, function and independence
